📝 Overview
Diagnoses can be assigned to a patient through multiple pathways within the system, including the patient's history, patient snapshot, or within a medical note. This article outlines each method and explains how to manage custom diagnoses for your practice.
🛠️ Methods for Assigning Diagnoses
There are a few ways to assign a new diagnoses to a patient. This section will cover how to do so.
Medical Note
Assigning diagnoses from a medical note is the most common method. Medical notes are typically created at check-in and diagnoses are added as the note is completed.
Navigate to the Assessment section of the medical note
You will see any open diagnoses, which can be closed, edited or deleted from the gear icon to the right of the diagnosis. To add a new diagnosis, click on the Add Diagnosis icon next to the Diagnoses section
Complete the form that appears
In the Diagnosis field, start typing and a list of all the diagnoses will appear in a drop down. The more you type, the system will narrow down what you need. Additionally, if you need to create a new diagnosis for your system, click Add New. Once created, this diagnosis will be available for future use.
💡 The diagnosis widget is included by default in the Basic, Simple, and Standard SOAP templates. Custom medical note templates must include the widget for diagnoses to display.
Patient History
You can assign a new diagnosis to a patient directly from the patient profile.
Navigate to the patient's profile
Click on the History tab, and select the More option. Click Diagnoses from the dropdown
Select the New Diagnosis button on the right
Complete the form that appears
Patient Snapshot
Navigate to the patient's profile
Click on the Overview tab
Scroll down to view the Snapshot view. The Open Diagnoses section will be on the bottom left of the snapshot
Click on the plus icon next to the Open Diagnoses section
Complete the form that appears
From the 3 dots button, diagnoses can be closed, edited or deleted.
⚙️ Managing Custom Diagnoses
If the diagnosis you need is not included in AAHA's diagnostic terms, you can create a custom diagnosis for use in your practice. Custom diagnoses are managed from the Patient Dashboard but can be added wherever a new diagnosis is assigned.
Adding a Custom Diagnosis
Click on the Patient module from the left hand navigation, and select Diagnoses
Click on Custom Diagnoses and select New Diagnosis
Complete the form that appears
Delete or Edit a Custom Diagnosis
A custom diagnosis can only be deleted prior to being used in a patient record.
Click on Custom Diagnoses and select View Active Diagnoses
Click on the gear icon to the right of the diagnosis and select delete or edit
Suspend a Custom Diagnosis
Once a custom diagnosis has been used in a patient record, it can no longer be deleted but can be deactivated to remove it from future diagnosis selections.
Click on Custom Diagnoses and select View Active Diagnoses
Click on the gear icon for the diagnosis you wish to deactivate
Select Suspend Diagnosis
